The goal is not to eliminate expression — it is to stop a hyperactive muscle from drawing a wrinkle that does not represent you. Dose, point and depth — the three details that separate a good result from a frozen face.
Botulinum toxin is a protein that temporarily blocks the signal between nerve and muscle. In aesthetic medicine it is used to relax hyperactive muscles that draw expression lines — glabella, forehead, crow's feet, chin, neck — or that break facial proportion (hypertrophic masseter).
It makes sense when a wrinkle is dynamic — appears with movement — and not yet so marked at rest as to need filler. Also as prevention in young patients with marked expression patterns. It does not make sense when the patient wants to eliminate all expression, nor when the wrinkle is static and deep.
I work with the leading botulinum toxin brands — Bocouture, Vistabel, Azzalure, Xeomin — and choose per case: there are real differences in onset speed, diffusion and patient sensitivity. I adapt dose and point to your specific muscular anatomy. Two patients of the same age with the same complaint may need very different plans.
I prefer under-correction to over-correction. A well-dosed toxin relaxes the wrinkle without erasing expression: you still frown, smile and express yourself, just with less noise. It can be reinforced at 2 weeks if needed; over-correction takes 3–4 months to reverse.
In the first visit I analyse your expression in movement — not only at rest — and decide which muscles to treat and at what dose. I show you where we will inject and what to expect.
Treatment is done in the same consultation and lasts 15–20 minutes. Very fine needle; discomfort is minimal and does not usually require topical anaesthetic. Effect sets in progressively between day 3 and day 14, with a follow-up visit at 2 weeks.
Effect is visible at 3–5 days and reaches full expression at 2 weeks. Duration: 3 to 5 months in the face; 4–5 months in the masseter; 6–9 months in axillary or palmar hyperhidrosis.
A well-indicated result feels natural: skin reads more rested, dynamic wrinkles are softened, but the expression is still yours. Duration extends with time: the muscle, using that pattern less, tends to stay relaxed longer between sessions.
When the patient wants a "frozen face" or absolutely smooth skin. It is not my aesthetic criterion and I will not do it, even though the technique allows it.
When the wrinkle is static and deep: toxin alone does not erase it — here the correct treatment usually combines toxin with another technique. When there are medical contraindications — neuromuscular diseases, pregnancy, breastfeeding, active infection — or expectations that do not match what the technique offers.
