Under-eye treatment in Barcelona
The word "dark circles" covers five different problems. The most common clinical mistake is treating them all the same way. Every consultation starts with a diagnosis of the type of under-eye issue you have.
What under-eye issues really are (and why they're not all the same)
The term "dark circles" groups very different realities: pigmentation (darkness from melanin or hemosiderin), hollowing (structural or tear trough), venous congestion (bluish tone from circulation), skin laxity, or fat pads (adipose hernia of the lower eyelid).
Each needs a different technique. Filling a pigmented under-eye with hyaluronic acid worsens the outcome; treating a structural hollow with laser changes nothing; and true fat pads are not solved with injections. The first step is diagnosing what type you have.
My approach
In consultation I observe the under-eye under different lighting and positions. I identify the primary component and secondary ones, and build a plan usually combining techniques: hyaluronic acid designed for tear trough (Restylane Vital Light, Teosyal Redensity II, Belotero Balance), polynucleotides (Plinest, Newest) for tissue quality, laser or peel for pigmentation.
For genuine fat pads, I refer for blepharoplasty — not everything is solved with injections. Knowing when NOT to inject is as important as knowing when to.
What the process looks like
The first visit is diagnostic. I show you what type or combination of under-eye issue you have and propose the plan with the minimum intervention possible.
If the plan includes hyaluronic acid, injection is performed with cannula to reduce vascular risk in this sensitive area. Session lasts 30 minutes, with topical anaesthetic. If it includes polynucleotides or mesotherapy, 3–4 sessions spaced every 2–3 weeks are typical.
What to expect
For structural under-eye, effect is immediate but the final result at 3–4 weeks. Duration: 12 to 24 months, one of the longest in aesthetic medicine. With polynucleotides, change is gradual and cumulative: perceived from the 2nd–3rd session.
A small bruise may appear in the first 48 hours — the area is highly vascularised — and resolves on its own. Important to consider if you have an event in the following days.
When I DO NOT recommend treating under-eyes
When the primary component is a marked fat pad. Here the correct treatment is surgery (blepharoplasty), not injections. Referral is part of clinical criterion.
When the skin is too thin and translucent: hyaluronic acid produces the well-known "Tyndall effect", a bluish tone visible under the skin. In these cases I choose polynucleotides or discard the treatment. And when the patient wants to eliminate under-eye area completely: reducing the impact, yes; erasing natural anatomy, no.
Frequently asked questions
How long does under-eye filler with hyaluronic acid last?
Between 12 and 24 months, one of the longest durations in injectable aesthetic medicine, thanks to the low mobility of the area.
What are polynucleotides for under-eyes?
DNA fragments of marine origin that stimulate under-eye skin regeneration: they improve texture, density and colour without adding volume. Ideal when the problem is not hollowing but skin quality.
Is under-eye treatment with hyaluronic acid dangerous?
In hands with specific training in this area, no. I use cannula to minimise vascular risk. It is a technically demanding area that should not be treated without specific experience.
Can bruising appear afterwards?
Yes, it can. The periocular area is highly vascularised and a small bruise — resolving in 4–7 days — falls within normality. We factor it in if you have an event coming up.
